- All co-pays and billed charges are due at the time services
are provided.
Although an insurance claim is being filed, you will receive
a monthly statement from my billing office if your account
has a balance due. We are happy to assist you with your
reimbursement from your insurance company but this office
cannot accept responsibility for collecting your insurance
claim or for negotiating a settlement on a disputed claim.
The patient or guardian is responsible for payment of the
account.
- I am a member of many Preferred Provider Organizations
(PPO) and Health Maintenance Organizations (HMO). As a participating
provider, we accept their payments in full, billing you
only for deductibles, co-insurance, co-payments and non-covered
items.
- If I am not a member of your insurance plan, please contact
our office in advance of any surgery to discuss your insurance
benefits and payment arrangements.
- If your plan requires authorization for the office visit
or procedures done in the office, this must be completed
when you arrive for the appointment. Your visit or procedure
may be canceled if the prior approval is not completed.
Please check with your insurance plan or your Primary Care
Provider (PCP) for help with this.
- Self-insured patients have to make arrangements prior
to their scheduled procedure. If you need to initiate a
payment plan, we encourage you to contact our office for
assistance in the management of your account.
- In the event that you require an operative procedure
to be performed, the office will bill your insurance company
for the operation only. If the insurance company has not
made a payment within sixty days from the date of the operation,
the balance becomes the patient's responsibility.
- For your convenience, the office accepts Visa, and Master
Card.
- Any balance outstanding after 90 days is " automatically"
forwarded to our collection service unless prior arrangements
have been made.
- Questions about your hospital, surgical center, or other
professional service bills (Anesthesiologist, Pathologist,
Radiologist, Assisting Surgeon) should be directed to their
respective billing department. We make an effort to use
Physicians participating in your plan, but cannot guarantee
the status of other providers with your insurance.
Please do not hesitate to contact our office in regards to
our charges, for assistance with your insurance, for payment
arrangements or any other concerns you may have.
