Being Prepared for your visit:

  • Please schedule an appointment before your visit to our office. To schedule an appointment, please call us at 407-647-3460.
  • Please bring your office visit medical insurance card, identification document and completed forms listed below. It is very helpful to bring your medication containers, any documents and radiological studies related to your surgical disease.
  • Fill out the forms regarding your health history and current health issues to the best of your ability. Try to answer all sections of your medical history. To download forms, please click here.
  • Have a complete list of all medications you are currently taking as well as any medication you are allergic to.
  • Be prepared. If necessary write down your symptoms so you won't leave anything out. This will help your doctor with his diagnosis and plan of treatment.
  • Ask questions at the beginning of your appointment. Your doctor may not have adequate time to discuss your concerns if you wait until the end of the appointment to mention them. Come to the office with your questions written down. This will help.
  • Be specific when describing your symptoms. For example: Not specific: My heartburn has gotten better. Specific: I get heartburn less frequently during the day but at night it still wakes me up two or three times a week.
  • If you are hearing or visually impaired, or can not speak English, you are encouraged to bring a family member or friend with you.

Understanding your healthcare provider:

  • Be sure you understand what your doctor is telling you by repeating it to him. You also can bring a note pad and write it down.
    Example: You said to take my prescription three times a day and you want me to change the dressing two times a day. Is that right?
  • Repeating the instructions also gives your doctor a chance to add anything you may have forgotten or to clarify his instructions.
  • Always follow your doctor's instructions for a speedy and healthy recovery.
  • The doctor's medical staff is also there for you to clarify and help you with your medical needs. Never leave the office if you have unanswered questions about your treatment.
  • Let your doctor know how you feel about your care. If you disagree with him, or the medication is not working, let him know. Likewise, let him know when you are happy about your medical progress and the care you have received.
