Confidentiality

All communication between a patient and the healthcare provider is protected by law. Release of information requires your written permission. Some exceptions may apply. Please review the Notice of Privacy Practices form as required by the Portability and Accountability Act of 1996 HIPAA.

Appointments

All services are by appointment. If a patient is unable to keep their appointment a 24-hour cancellation is appreciated. If you are a no-show or miss your appointment, our office will attempt to contact you by phone to reschedule the visit. Your appointment time represents a valuable time for you and your doctor. Due to unplanned emergencies there may be times that your doctor is running a few minutes late. If your wait time exceeds thirty minutes you will be offered a chance to reschedule.

Patient Registration

New patients will be asked to fill out a complete form related to his or her medical history. This information will assist the doctor with important information regarding the background of your problem and any previous treatment. Completion of this questionnaire gives you a chance to organize your thoughts and tell the doctor about the symptoms, history, and treatment of the current complaint and past illnesses. Please allow yourself at least (15) minutes time to fill out all documents. It is very helpful to have your complete list of medications at the time of the visit.

Office Hours of Operation

Monday through Thursday 8:30AM to 5:00PM and Friday 8:30AM to 4:00PM.

Telephone Calls and Messages

As your call is important to us, telephone messages left on the answering machine will be returned as soon as possible during the weekdays and the next business day after weekends and holidays. Within normal office hours all calls will be answered and returned in a timely fashion by the staff and Dr. Dobradin.

Referral Doctors

The primary care physician and/or referring physician will receive medical notes and/or reports from our office.

Cancer Patients

All cancer patients will be expected to follow up with us every three months the first year, every six months the second year, and annually after that for up to five years.

Breast Cancer Patients

The breast cancer patients are expected to bring their mammogram, ultrasound and/or MRI films for initial and annual follow-up visits.

We encourage patients to follow current National Cancer Institute breast screening guidance. Our policy is to interpret radiological and pathological reports only during the time of you office visit. We can release your medical information but interpretation of such will be available only during the face-to-face office contract. Please arrange a follow-up visit one week after the imaging study and make sure that you bring the films with you.

Surgical Care

Post-operative care is available during office hours. In the event of an after hours emergency, please listen to the message on our answering machine for instructions. If you need immediate assistance, please visit the nearest emergency department or call 911.

  • When Dr. Dobradin is absent, such as during a vacation, it may be necessary for us to make arrangement with another physician.

Surgical Procedures

In complex surgical operations a second surgeon may be required to assist. You will receive additional billing for those services.

Medical Tests Results

Dr. Dobradin will give you the results for all medical tests ordered. Please make sure you schedule this follow up appointment. At that time any questions or concerns needing to be answered can be addressed as well as the treatment plan.

Treatment Room

For procedures that are done in the treatment room, we ask for safety reasons, that you do not bring children into the room with you.

Medical Forms to Be Filled Out

Completion of all forms related to disability or others(insurance, legal, etc) take additional time outside of your treatment. Preparation of these forms is not covered by your insurance company. You will be charged $25.00 for one page, $50.00 for two pages or $75.00 for three or more pages. Some exceptions may apply. For return to work and/or school there is no charge.
